Summer Camp Registration

 I ,  give permission for my child to be transported to and/or from Sah Do Mu Sul (SDMS),  929 North Main St. O’Fallon, MO 63366) by vehicle operated by George or Kelly Fleites for the purposes of after school pick up or field trip destinations. 

I/we release SDMS, its officers, employees, staff, and volunteers from any and all liability of any kind whatsoever for any loss or injury to my/our child arising from riding in the bus/van and participating in SDMS activities. I/we agree to indemnify and hold forever harmless SDMS, its officers, employees, staff, and volunteers from any and all liability of any kind whatsoever for loss or injury to my/our child arising from riding on the bus and participating in the SDMS after school pick up or field trip.

Photograph Release:

I authorize the Sah Do Mu Sul LLC to use photos, and or other likenesses of myself and or my child or the child for whom I have legal guardianship for any promotional materials in paper and online regarding Sah Do Mu Sul LLC brochures, programs, facilities, or services. Such likenesses will not be sold to other parties. Promotional materials bearing these likenesses may be distributed for free to the public and posted on the Sah Do Mu Sul LLC website. Sah Do Mu Sul LLC reserves the right to use any photo or likeness for a time period beginning when this form is signed and ending upon written request of participant, parent or legal guardian.

SDMS Waiver:

I/Student, the parent, or legal guardian on behalf of the student, acknowledges and is fully cognizant of all the inherent dangers of Martial Arts and acknowledges that the execution of Martial Arts is a vigorous, physically-demanding activity with contact. Student realizes that there is a risk of physical injury which may be incurred while engaged in this activity. Student represents that he or she is physically fit to participate in Martial Arts instruction and, if necessary, has consulted a medical doctor. Student has had the opportunity to observe and participate in Sah Do Mu Sul LLC Martial Arts prior to signing this agreement. Student, parent, or legal guardian on behalf of the student, hereby waives any and all claims for any personal injury, damage, or loss in connection with the training at the School and expressly assumes the risk of all dangers or injury inherent to the Martial Arts. Student understands that Sah Do Mu Sul LLC employees, other authorized persons, and students will be engaged in physical contact and Student gives informed consent to such contact. Further, the School does not warrant, implied or otherwise, the safety of any practice equipment, including mats and sparring gear, weapons, or any other martial art supplies.

Installment Payment Method / Authorization for Payment

The Buyer hereby authorizes the company Sah Do Mu LLC or its agents and/or assigns to electronically deduct/process on or after the due date for tuition payment directly from the following bank or credit/debit card accounts according to the terms of this agreement and to resubmit returned/declined items without prior notice. This payment information will be used to collect any balances past due according to the terms.
Buyer acknowledges that Sah Do Mu Sul LLC utilizes the services of a third-party payment processing collection company, balance will be reported to credit bureau. Seller's liability for any payments wrongfully processed or collected shall be solely limited to such wrongful amount, and to the extent Buyer suffers damages in excess of such amount, whether bank charges, attorney's fees or other costs or expenses, Buyer agrees to hold harmless and indemnify Seller from same. Approved refunds shall be paid with 20 days after receipt of the notice of such wrongful charge collected by Seller. Buyer also agrees not to back charge any transactions to their credit/debit card company.

ALL SALES ARE FINAL AND NONREFUNDABLE.
